Templates for forms can be found on sites like Pixel Perfect, Adoxography, or Terrible (if you don't want to make an account). Otherwise, you can look at other peoples' templates and ask for permission to use them. As for learning to code, I'd recommend starting from scratch on w3schools, or using resources from Pixel Perfect and Adoxography.
